Jax Surf and Paddle Forms

Anyone joining a Jax Surf and Paddle Class, Lesson, Surf Camp, Activity, or Using Rental Equipment must sign a  Release of Liability form in order to participate. For children under 18 a legal guardian must sign release. Please sign and bring your completed Release of Liability to your session.
